          Advantage Afterschool Program Forms

          The Advantage After School Program is a homework, social and recreational program for Amherst Middle School students. The program is located at Amherst Middle School and runs Monday through Friday 3:30 PM - 6:30 PM. This program begins in September and runs through the school year.

          A letter to parents is below, and required registration forms are at the end of this page.

          Dear Families and Friends,

          We would like to update you on the 2011-2012 Advantage Afterschool Program at Amherst Middle School. The program will begin this year on the first day of school. Program will be held Monday – Friday, 3:45 pm – 6:00 pm; we do not hold program on the days that we have a 12:40 pm dismissal.

          It is with great enthusiasm that I introduce myself, Julianna Zippiroli, as Program Director at the Advantage Afterschool Program. I have had the pleasure of working with the Town of Amherst for the past eight years. The counselors and I are looking forward to a great school year filled with lots of studying, activities and fun! Please read this letter carefully, as it contains important information regarding after school care.

          1. DAILY PROGRAM:
          A) Students have 15 minutes from dismissal (3:45 pm) to go to their lockers and get their belongings and check in for attendance for program. The students must be in the cafeteria by 4:00 pm. If they are staying afterschool for a sport, club, detention, or with a teacher, they MUST come and let us know before they go.

          B) We do provide daily snacks, see monthly snack calendar.

          C) One main focus of our program is homework. If you would like your child to receive extra assistance with homework or would like to make sure their homework is complete prior to other activities, please speak to the Program Director. After snack and announcements, we have Homework/Quiet Time until 5:00 pm. Students must do homework, study or find something quiet to do during this time. We also have certified teachers on staff that will be assisting with homework.

          D) If students are done with homework they have a choice to do, or not do, the planned activity. Planned activities include a focus on selfesteem, good decisionmaking and responsible behavior as well as guest speakers, community service projects, crafts, cooking, team building activities, board games, and ping pong. Also, whenever available and/or weather permitting, we go to the gym or outside for organized sports and free time. We do our best to make constructive use of the time and make it fun and enjoyable for all of the children participating.

          2. PICK-UP:
          1. A) We are located in the school cafeteria, right across from the main office. ALL students must be picked up by 6:00 pm. There will be a $5.00/15 minute/child late fee for any student picked up after 6:00pm. You must personally signout your child every day. We ask that all students be ESCORTED out, so you may be aware of any behavior issues or schedule/program changes. Please remember, you must provide Photo ID on any given day, if requested by staff until we are familiar with parents/guardians or whoever may be picking up your child. This is for your child’s safety. We cannot release a child without proper ID and permission (pick-up list) from the parent. The program director must be notified of any situation(s) in which someone other than the parent(s) will be picking up your child, who is NOT on the pick-up list. This person must bring their driver’s license or photo ID as well. If you have any concerns regarding the pick-up of your child, please speak with the Program Director.

          B) If your child is walking home, they need to have a note from a parent or guardian (to be kept on file) granting them permission to walk home and the time that they are allowed to leave. Students walking home MUST leave by 6:00 pm or before it gets dark.

          C) Students in the Advantage program are also able to take the late bus, they must let the staff know during attendance; this bus leaves approximately at 4:30 pm. Late buses do not run on Mondays and Fridays, early dismissal days or days before long weekends/breaks.

          3. BEHAVIOR PLAN:
          Our goal is for the students to be successful and responsible. We have created “Advantage Expectations”, which are the program rules that the students need to follow in order to have a fun and safe time at program. Counselors will promote these expectations throughout the school year with positive reinforcement. To compliment “Advantage Expectations”, we also have the “The Advantage Tickets” which promotes not only positive behavior but also takes in account their homework effort too! At the end of each month, students will be able to turn in their tickets for prizes and privileges. The students that qualify each month will also have the opportunity to go on a field trip at the end of the year.

          4. BILLING:
          The daily program fee is $3.00 per student. If for whatever reason you are unable to afford that, please see the program director about possible scholarship money. There will be a $40.00 deposit for this program ($20.00 of this deposit will be applied to your June billing statement). The deposit will be reflected on your first billing statement. Billing is done on a monthly basis. You will be charged for the days you have registered your child. All payments are due by the 30th of each month. Any late payment will incur a $ 10.00 late charge. If full payment is not made by the 1st of the following month your child will be sent home on the regular bus and removed from program If your child is absent and our program is notified by 3:30 pm you will receive credit as follows:

          **This program does not operate on an as needed basis. Julie Krzyzanowski is our program’s clerical and billing person. All payments can be dropped off at the Amherst Youth & Recreation Office or mailed at the address listed on the letter head. Checks should be made payable to AMHERST YOUTH FOUNDATION.

          **Please include your child’s name in the memo section of the check. We are now offering the use of Credit Cards as a form of payment. If you would like us to keep your credit card on file, there is a form you may fill out for this. Your account will be charged your tuition amount on the 30th of each month. If you have any billing questions you may call 631-7132 ext. 7543 to leave a message for Julie.

          5. POLICIES AND PROCEDURES:
          A) If your child will be absent from our program, please call 362-7189 BEFORE program starts to inform us of the absence.

          B) Cell phones are NOT allowed out during program.

          C) Our Program Director and at least two other counselors on site are trained in First Aid and CPR. It is our policy to notify you immediately of any first aid that involves your child. If the parent(s) cannot be reached, we will use the emergency numbers that you have provided. Please ensure that ALL phone numbers are accurate and current.

          D) In accordance with state regulations, if your child has contracted a contagious condition, we MUST be notified immediately (i.e.: strep throat, lice, pink eye, etc.)

          E) If your child requires medication, that information must be given to the Program Director personally. All medication must be in its original container and a medical consent form must be completed by the parent and Health Care Provider before the program will accept any medication.
